Understanding PET Scans: How They Work and What They Show

A PET scan, short for Positron Emission Tomography scan, is a type of medical imaging that helps doctors see how your body's cells are working. Unlike X-rays that show the structure of bones and organs, PET scans reveal the function and activity level of cells throughout your body. This guide explains how PET scans operate and why doctors order them for patients.

During a PET scan, a small amount of radioactive substance called a tracer is injected into your bloodstream, swallowed, or inhaled depending on what part of your body needs imaging. This tracer contains glucose (a type of sugar) or other compounds that your body's cells use for energy. Cancer cells, areas of inflammation, and other abnormal tissues typically use more glucose than healthy tissue, so the tracer accumulates in these areas.

A special camera called a PET scanner detects the energy signals released by the tracer as it moves through your body. These signals are converted into detailed images that show where the tracer is concentrated. Areas of high tracer concentration appear as bright spots on the image. A computer then creates three-dimensional pictures that doctors can examine from different angles.

Many PET scans are combined with CT scans in a procedure called a PET-CT scan. The CT portion provides a clear structural image of your organs and tissues, while the PET portion shows cellular activity. Together, these images give doctors a complete picture of both how your body looks and how it is functioning at a cellular level. This combination helps doctors pinpoint exactly where abnormalities are located.

The entire PET scan process typically takes 30 to 60 minutes from start to finish, though the actual scanning time is usually 20 to 30 minutes. Most facilities require patients to arrive early for check-in and preparation. The tracer is administered about 45 to 60 minutes before scanning begins, allowing time for it to distribute throughout your body. During the scan itself, you must remain very still while lying on a table that slides in and out of the scanner.

Practical takeaway: PET scans show doctors how your cells are functioning by tracking the movement of a radioactive tracer through your body. Understanding this basic process helps you know what to expect during your appointment and why your doctor may have ordered this type of imaging.

Common Medical Reasons Doctors Order PET Scans

Doctors order PET scans for many different medical reasons. The most common use is to detect and monitor cancer. According to the American Cancer Society, PET scans can identify cancerous tumors, show whether cancer has spread to other parts of the body, and help doctors determine whether previous cancer treatment was successful. PET scans are frequently used for patients with lung cancer, lymphoma, melanoma, and breast cancer, among other cancer types.

Beyond cancer detection, PET scans are valuable for evaluating heart disease and brain disorders. For cardiac patients, PET scans can show which areas of the heart muscle have reduced blood flow and may be at risk. This information helps cardiologists decide whether a patient needs interventions like angioplasty or bypass surgery. Studies show that PET scans can accurately predict which heart patients are most likely to benefit from revascularization procedures.

In neurology, PET scans help diagnose and monitor conditions like Alzheimer's disease, Parkinson's disease, and epilepsy. A PET scan can reveal patterns of brain activity that suggest Alzheimer's disease even before memory loss becomes noticeable. For epilepsy patients, PET scans can identify the specific area of the brain where seizures originate, which is crucial information for surgical planning. Neurologists also use PET scans to evaluate patients with unexplained cognitive decline or behavioral changes.

PET scans are also ordered to evaluate infections, assess bone disease, and investigate fever of unknown origin. In patients with chronic infections like tuberculosis or endocarditis, PET scans can show the extent and location of infection throughout the body. Orthopedic surgeons sometimes use PET scans to evaluate complicated bone problems or to assess the viability of bone grafts after surgery.

The specific type of tracer used in your PET scan depends on what your doctor is investigating. Fluorodeoxyglucose, or FDG, is the most commonly used tracer and works well for cancer detection because cancer cells consume glucose rapidly. Other tracers are better suited for cardiac or neurological imaging. Your doctor will select the appropriate tracer based on your medical condition and the information needed for diagnosis or treatment planning.

Practical takeaway: PET scans serve many purposes in modern medicine, from cancer detection to heart disease evaluation to brain disorder assessment. Knowing why your doctor ordered a PET scan helps you understand what medical question the scan is designed to answer.

Preparing for Your PET Scan Appointment

Proper preparation for a PET scan helps ensure clear images and accurate results. Your doctor's office should provide you with detailed preparation instructions specific to your scan. While requirements vary depending on the type of PET scan you're having, several general preparation guidelines apply to most patients. Following these instructions carefully is important for getting reliable results.

Fasting before your scan is typically required. Most patients need to fast for four to six hours before their appointment, meaning no food or beverages except water. This fasting period is necessary because food interferes with how the tracer distributes throughout your body. If you have diabetes, your doctor will give you specific fasting instructions and may adjust your insulin or diabetes medications. Always discuss your diabetes management with your imaging center in advance if this applies to you.

Medication management is another important preparation aspect. Bring a complete list of all medications and supplements you take to your appointment. Some medications can interfere with PET scan results. For example, certain diabetes medications may need to be held before your scan. Blood sugar levels should be controlled and as close to normal as possible before scanning because elevated glucose levels can affect image quality. Your doctor will advise whether to take your regular medications on the day of your scan.

Wear comfortable, loose-fitting clothing without metal buttons, zippers, or snaps. Metal objects can interfere with imaging, so clothing that doesn't contain metal makes the process smoother. Leave jewelry, watches, and other metal items at home. If you wear a pacemaker or have had metal implants, inform the imaging facility in advance. Most modern implants are compatible with PET-CT scans, but the facility needs to know about them beforehand.

Arrive early for your appointment, typically 15 to 20 minutes before your scheduled scan time. You'll complete registration paperwork and may have a brief pre-scan consultation with a technician who will explain the procedure and answer your questions. They'll ask about recent illnesses, medications, surgeries, and any implanted devices. This information helps the technician ensure your safety and helps the radiologist interpret your images accurately. Bring your insurance card and photo identification.

Plan to be at the imaging facility for about two to three hours total. After the tracer injection, you'll spend time in a quiet waiting area while it distributes through your body. During this waiting period, avoid talking and unnecessary movement because physical activity affects tracer distribution. Some facilities have quiet rooms where you can rest during this waiting period. Many patients bring headphones or find the waiting time a good opportunity to read or rest.

Practical takeaway: Proper preparation significantly improves scan quality and accuracy. Following your doctor's specific instructions about fasting, medications, and arrival time helps ensure your PET scan provides the information your doctor needs for diagnosis or treatment planning.

What to Expect During and After Your PET Scan

Understanding what happens during the actual scanning process reduces anxiety and helps you know how to cooperate with the technician. When you enter the scanning room, you'll lie on a padded table in a comfortable position. The technician will help position you correctly and may use pillows or supports to help you stay still. Remaining motionless during the scan is crucial because any movement can blur the images and reduce their quality. The scanning table will then slide into the scanner opening.

The PET scanner looks like a large tube with a tunnel through the middle. The opening is large enough that most patients feel comfortable, though some people experience mild claustrophobia. The actual scanning time typically lasts 20 to 30 minutes, though you may be in the scanner room for longer. During scanning, you'll hear soft humming and clicking sounds as the equipment operates. These sounds are normal and
