Understanding Rotator Cuff Tears: What Happens to Your Shoulder

The rotator cuff is a group of four muscles and tendons that work together to hold your shoulder joint in place and allow you to move your arm. These muscles are called the supraspinatus, infraspinatus, teres minor, and subscapularis. When one or more of these tendons tear, it creates what doctors call a rotator cuff tear. This injury can happen suddenly, like when you fall on your shoulder, or it can develop slowly over time from wear and tear.

Rotator cuff tears are quite common. Research shows that about 20 percent of people over age 60 have a rotator cuff tear, though not all of them experience pain or symptoms. In younger people, tears usually result from a specific injury or accident. In older adults, tears often happen because the tendons naturally weaken with age and repeated use.

There are different types of rotator cuff tears. A partial tear means the tendon is damaged but not completely separated from the bone. A full-thickness tear goes all the way through the tendon. Some tears are small, while others are large and affect multiple tendons. The size and location of the tear determine how serious it is and what treatment options may work best.

Common symptoms of a rotator cuff tear include pain in the shoulder, especially at night or when moving your arm in certain directions, weakness when lifting or rotating your arm, a crackling sensation when moving your shoulder, and difficulty reaching behind your back. Not everyone with a rotator cuff tear has symptoms. Some people discover they have a tear only when getting imaging done for a different reason.

Practical takeaway: Understanding that rotator cuff tears range from minor to severe helps explain why recovery plans vary from person to person. A small tear in a younger person may heal differently than a large tear in an older adult. Knowing what your specific tear looks like—information your doctor can provide through imaging—helps you understand what to expect in your recovery.

The Early Stages of Recovery: Rest and Protection

The first phase of rotator cuff tear recovery typically focuses on reducing pain and inflammation while protecting the injured tendon from further damage. This phase usually lasts two to four weeks, depending on the severity of your tear and your body's response to injury. During this time, your main goals are to rest your shoulder, use ice to reduce swelling, and take steps to prevent re-injury.

Rest doesn't mean complete immobility. Instead, it means avoiding activities that aggravate your shoulder. Many people find it helpful to wear a sling to limit shoulder movement and remind themselves not to use the injured arm. Your doctor may recommend that you sleep in a specific position to avoid putting pressure on the injured shoulder. Some people find that sleeping on their uninjured side with a pillow between their arms feels more comfortable.

Ice therapy can help manage pain and reduce inflammation in the early stages. Applying ice for 15 to 20 minutes several times a day may help, especially after any activity that increases pain. Some people alternate between ice and heat, though ice is generally more useful in the first few days after injury.

During this early phase, anti-inflammatory medications such as ibuprofen or naproxen may help reduce pain and swelling. Your doctor can recommend whether these medications are appropriate for you based on your overall health and any other conditions you have. Some people benefit from steroid injections into the shoulder joint to reduce inflammation and pain, though this is a decision to discuss with your healthcare provider.

Physical therapy often begins even in this early phase, but the focus is gentle. A physical therapist might guide you through passive range-of-motion exercises, where they move your arm for you without you doing the work. This helps maintain some movement capability without stressing the healing tendon.

Practical takeaway: The early recovery phase is about patience and protection. During this time, focus on following your doctor's recommendations about rest, ice, and any medications. The goal is to let the inflammation settle so you can progress to more active exercises without making the tear worse.

Progressive Physical Therapy: Building Strength and Range of Motion

As your shoulder begins to heal, usually after the first four to six weeks, recovery shifts toward active rehabilitation. This phase focuses on gradually increasing your shoulder's range of motion and strength. Physical therapy becomes more central to your recovery plan. A licensed physical therapist designs exercises tailored to your specific tear and recovery stage.

Range-of-motion exercises come first in this phase. These exercises help restore your shoulder's ability to move in all directions. Examples include pendulum exercises, where you bend forward at the waist and let your injured arm hang down, then gently swing it in small circles. Another common exercise is the wall walk, where you use your fingers to "walk" up a wall, gradually reaching higher to increase shoulder flexion. These exercises should feel manageable—you should not push through significant pain.

Strengthening exercises begin as your pain decreases and your range of motion improves. These exercises target the rotator cuff muscles specifically and also work the surrounding shoulder muscles that support and stabilize the joint. Common strengthening exercises include resistance band work, where you use elastic bands to provide gentle resistance as you move your arm through different motions. Your physical therapist will show you proper form to ensure you're activating the correct muscles.

The pace of progression varies significantly based on several factors. Younger people typically progress faster than older adults. Small tears generally heal faster than large tears. A tear that happened suddenly may follow a different timeline than a tear that developed gradually over months or years. Most physical therapists follow a general guideline of increasing difficulty gradually—typically adding new exercises or more resistance every two to three weeks as you demonstrate capability and reduced pain.

Research indicates that consistent physical therapy participation strongly influences recovery outcomes. People who attend regular sessions and do home exercises between sessions tend to recover better and faster than those who don't follow through with therapy. Most recovery programs involve three to four physical therapy sessions per week initially, potentially reducing to once or twice weekly as recovery progresses.

Practical takeaway: Think of physical therapy as a gradual journey with clear milestones. You won't jump from gentle passive motion to heavy strengthening. Each exercise you master builds the foundation for the next level. Consistency matters more than intensity—doing your prescribed exercises regularly, even if they seem simple, produces better results than occasional intense sessions.

Surgical Options and Recovery: When Conservative Treatment Isn't Enough

Some rotator cuff tears don't heal adequately with rest and physical therapy alone. Surgery may become an option when conservative treatment fails, when the tear is large, when multiple tendons are involved, or when the tear happened suddenly from an injury in a younger, active person. Understanding surgical options helps you have informed conversations with your doctor about your recovery path.

The most common surgical approach for rotator cuff repair is arthroscopic surgery, which is minimally invasive. The surgeon makes small incisions and uses a camera to see inside the shoulder joint. They then reattach the torn tendon to the bone using small anchors and sutures. This approach typically causes less tissue damage, less pain after surgery, and faster recovery than traditional open surgery. However, not all tears can be repaired arthroscopically—large or complex tears sometimes require traditional open surgery through a larger incision.

Recovery from rotator cuff surgery typically follows a more structured timeline than non-surgical recovery. The first phase, lasting four to six weeks, focuses on protection and preventing re-injury to the repair. Your arm may be immobilized in a sling, and you'll avoid all active shoulder movement. Passive range-of-motion exercises, done by a therapist or with assistance, begin during this phase.

The second phase, from weeks six to twelve, introduces active-assisted exercises where you do some of the work with your muscles while the therapist provides support. Gentle strengthening begins. The third phase, from three to six months, progresses strengthening as the repair site becomes stronger. Return to normal activities typically takes four to six months, though heavy lifting and overhead activities may take six to twelve months to return to safely.

Post-surgical physical therapy is usually more intensive than non-surgical therapy, often involving three to four sessions weekly for several months. The surgeon's specific instructions about movement restrictions are important to follow carefully, as early aggressive movement can damage the repair. However, appropriate movement during the healing phases actually promotes better outcomes by stimulating the tissue to heal properly and regain function.

Practical takeaway: Surgery is not always
